Proxy Access


You may be granted proxy access to another individual's myLoyola account under the following conditions:

Child Under Age 12
A parent or legal quardian may request proxy access to the myLoyola account of a minor child under the age of 12 by completing the Minor Proxy Access Form and returning it to LUHS.
This type of proxy access will be revoked automatically when the minor child reaches the age of 12.

  • Child Between Age's 12 and 18
    Individuals over the age of 12 can grant proxy access to their myLoyola account to parents or legal guardians by completing the Proxy Access Consent Form and returning it to LUHS.

  • Adult Over Age 18
    Individuals over the age of 18 can grant proxy access to their myLoyola account to spouses,partners, family members, care givers or other designated individuals by completing the Proxy Access Consent Form and returning it to LUHS.
    The individual granting proxy access can revoke that proxy access at anytime for any reason.

  • Adult with Diminished Capacity (Care Giver Access)
    Adults who have a surrogate relationship with another adult through an Advance Directive, Power of Attorney or other legal arrangement may sign up for access to that person’s myLoyola account. Legal documentation is required to grant this type of proxy access.
